The Rehabilitation Institute of Chicago SYSTEMS CASE MANAGER Part-time - 20 hours per week Benefits eligible! The Rehabilitation Institute of Chicago, ranked the #1 rehab hospital for ten years consecutively, has an opportunity for a part-time Systems Case Manager who will be liaison to the interdisciplinary team to facilitate a smooth transition for the RIC patient from hospital to discharge destination. You will perform evaluations of clinical and financial information in order to form a management plan. Coordinate and facilitate access and service delivery for the patient ensuring a smooth transition from one episode of care to the next. Monitor and evaluate the services and resources received by the patient on an ongoing basis. Will also implement plan modifications as necessary and act as a resource to patient/family, physicians, and community agencies. Requirements include three years in rehabilitation with a working knowledge of all levels of rehab services. Two years in a managed care environment. CCM preferred. Knowledge of healthcare delivery systems and reimbursement practices. A Bachelor's Degree in Nursing or Social Work with licensure and/or certification in one's own discipline. ***Knowledge of disability case management is a huge plus!
